We are looking for a registered POH-S with experience in general practice. The ideal candidate is passionate about primary care, has a proactive and collaborative attitude, and is able to motivate patients to make lifestyle and behavioral changes. You are independent but also value collaboration within a multidisciplinary team and are available for 3 days a week.
